Health Care for the Homeless Co- Applicant Governing Board
The mission of the Health Care for the Homeless Co-Applicant Board is to oversee, guide and assist the program in its efforts to deliver high-quality health care to a diverse and medically underserved community. The Health Care for the Homeless co-applicant board meetings focus on governance and oversight of the Health Care for the Homeless program and services, partners, processes and policy.
The council meets on the third Wednesday of every month at 1220 Morello Ave. Martinez, CA 94553 on the 2nd floor in the "Walker" room from 11:00-12:30 p.m. All our meetings are open to the public. Stephen Krank, The Society of St. Vincent de Paul
Stephen's experience in the Social Service and nonprofit sector have ranged from Project Coordinator of a 432-bed IPTC (In Prison Therapeutic Community) the management of 170-bed social model detoxification and recovery program for street alcoholics and the start, program development through licensure of a 300-bed facility. In 2002 Stephen became involved the UCSF Community Health Project, and later joined the board as the program became The Street Level Health Project. The organization's substantial efforts to reach out and provide the systemic support needed to the underserved through the provision of resources and navigation to the most vulnerable; the recent immigrants in Fruitvale and beyond. For the last twenty years, Stephen has been part of The Society of St. Vincent de Paul in positions ranging from program development of the Champion Guidance Center to his current position as Director of Vincentian Support for the 29 parish-based branches of the Society in Contra Costa County.

Daisy Gallegos, Adult Protective Services
Daisy was born and raised in Contra Costa County and is currently a Senior Staff Assistant with Adult Protective Services. With APS, she manages several grants, including the Home Safe Program. Home Safe is designed to prevent or address homelessness, while establishing housing stability to the elder and dependent adult population. Prior to APS, Daisy was employed at La Clinica De La Raza assisting patients enroll into health insurances. Additionally, she was a program manager for an Independent Living Service agency and a Youth Program Coordinator at a community center. With these community organizations, Daisy was able to advocate for her clients and ensure they were receiving proper supportive services. Her passion is to continue to support my community and help design quality resources for the underserved population.

Claude Battaglia, Independent Living Specialist Claude Battaglia works for Choice in Aging (CIA) as the Transitions Services Manager. He oversees the CCT Program (California Community Transitions). We assist our consumers in transitioning from Skilled Nursing Facilities back into the community. We assist in providing them with home set up, Assistive Technology and getting them connected to care givers. What Claude likes best about working for CIA is seeing consumers faces light up when they get approved for housing and services. Originally from Pittsburg, California. Claude enjoys attending bay area sporting events, rooting for the Raiders, A's, Giants & Warriors and enjoys going to concerts.

Michael Callanan, CORE Outreach Coordinator Michael Callanan is the current Program Manager for the Contra Cosa County Health Housing and Homeless (H3) Services, CORE (Coordinated Outreach Referral and Engagement) Program. Michael was asked to help build the team in January of 2017 and he became the first CORE Program Coordinator. Prior to CORE, Michael was working also for H3 for the Contra Costa Adult Continuum of Services (CCACS) Program as the Site Supervisor at the Concord and Respite shelters for 2 years. Michael focuses on meeting people where they are at and likes to instill trust, hope and worth. He has been with H3 for 10 years and is certified in Addiction Treatment Counseling.

Bill Jones, Hope Solutions Bill Jones is a licensed marriage and family therapist and certified end of life counselor who has provided direct services to underserved populations for more than 20 years. He has worked as a case manager, mental health provider, clinical supervisor, and support group facilitator, and he has developed and managed mental health, case management, housing advocacy and permanent supportive housing programs. He currently serves as the Director of Properties and Support Services for Hope Solutions while maintaining a private psychotherapy practice. He lives in Central Contra Costa County, and he is honored to serve on the Health Care for the Homeless Co-Applicant Board.

Jennifer Machado, Chervon Federal Credit Union Board Member & Member of Walnut Creek St. Paul's Commons low income housing project Jennifer Machado is a finance executive with decades of experience in the corporate world and several non-profit organizations. Jennifer was a member of St. Paul's Episcopal Church's project team, the group that partnered with Resources for Community Development who built St. Paul's Commons on church property in Walnut Creek. St. Paul's Commons, which opened in 2020, is a complex of 45 units of affordable housing, meeting rooms, and space for day programs of the Trinity Center, an organization serving people experiencing homelessness. Jennifer is a supporter and volunteer in organizations promoting social justice, economic empowerment, and education.

Rachna Pandya, MPH, Strategic Implementation Leader Rachna is a resident of Danville with her family and has over 20 years' experience in healthcare and currently works at Kaiser Permanente Northern CA headquarters as a Strategic Implementation Leader. Her expertise is in leading teams in transformative design, pipeline development, and execution of strategic programs for vulnerable populations. She has worked in direct patient care, managed families in health coaching with chronic disease management, performance improvement efforts, and advocating for patient rights. Currently she has been working with Leadership and innovating programs across clinical care, health plan & hospital operations for complex care populations. Her strengths are putting ideas into actions across cross functional teams from idea to executing across Leadership and enterprise-wide strategy lens with social determinants of health as a priority. Rachna is honored to give back to her community given her healthcare experiences and has a passion both professionally and personally to serve others on the Health Care for the Homeless Co-applicant Board.
