Call CCHP Member Services at 1-877-661-6230 (Option 2).  Ask for “Continuity of Care.”

Unless you recently transitioned from Anthem Blue Cross Medi-Cal, we will need the medical records from the last 3 visits with your doctor.

Do NOT pay the bill.  Make sure the provider has your CCHP information so they can bill us.  Call CCHP Member Services right away if you need help!

Medi-Cal 

If you have Medi-Cal and are filling or refilling a prescription, you must get your prescribed drugs from a pharmacy that works with Medi-Cal Rx. You can find a list of pharmacies that work with Medi-Cal Rx on the Medi-Cal Rx website. 

 You can also find a pharmacy near you or a pharmacy that can mail your prescription to you by calling Medi-Cal Rx at 800-977-2273 (TTY 800-977-2273 and press 5 or 711).

Once you choose a pharmacy, take your prescription to the pharmacy. Your provider may also send it to the pharmacy for you. Give the pharmacy your prescription with your Medi-Cal Benefits Identification Card (BIC).

Commercial Plans 

If you have County Plan A, County Plan B, County Plan A2, or IHSS Plan A2, you must use a pharmacy in CCHP's network.  You can search for CCHP pharmacies under the Facility tab here.
