Care Services for People with HIV and Their Families

AIDS Drug Assistance Program (ADAP)

ADAP supports clients by paying for HIV-related medications. To participate, you must be:

• HIV positive
• A resident of California
• Over the age of 18
• Have limited or no prescription benefits
• Meet financial eligibility requirements

OA-Health Insurance Premium Payment (OA-HIPP)

• This program pays the monthly health insurance premiums for eligible California residents with an HIV/AIDS diagnosis.
• The program is available to people who are at risk of losing their health insurance. It is also available to people who don't have health insurance but would like to purchase it.
• You can stay in the program for an unlimited amount of time as long as you meet the following requirements:
  • Are enrolled in ADAP
  • Have an HIV/AIDS diagnosis
  • A California resident
  • At least 18 years old
  • Have an adjusted gross income below 500% federal poverty limit
  • NOT enrolled in Medicare or full-scope (free) Medi-Cal
  • Have (or plan to get) a health insurance plan with prescription drug benefits

Medical social workers can help you apply and can submit your application to the Office of AIDS (OA). Please contact the medical social worker of the day at 925-608-5384.

If you are undocumented and can't get medical insurance independently, OA-HIPP can help you get a California Department of Public Health off-exchange medical insurance program.

Early Intervention Services

These services help people live healthier and longer.

• Friendly and knowledgeable health education staff help clients learn how to:
  • Reduce the risk of transmitting HIV to sexual partners
  • Avoid getting other STIs
  • Remain confident in their health
• Medical social workers, outreach workers and nurses work as a team to help patients:
  • Make and keep appointments
  • Understand their medications
  • Access clinical services.
• Physicians can provide specialty appointments. They're knowledgeable and experienced in the latest HIV treatment guidelines.

Other early intervention services include:

• Information and Education around Pre-Exposure Prophylaxis (PrEP)
  • PrEP is a once-daily prescription pill taken by an HIV-negative person. It reduces their risk of HIV infection. The PrEP medication, called Truvada, is very effective at preventing HIV when taken correctly.
• Enrollment into the PrEP Drug Assistance Program (PrEP-AP)
  • This program assists eligible people in paying insurance costs for PrEP.
• Free condoms and lubricant to reduce risk for HIV and other STDs.

Outreach and Linkage to Care

Our dedicated teams connect people to medical care and case management. They provide outreach to make sure all people living with HIV are getting the care they need. This is helpful for:

• People who have never been connected to medical care
• People who have fallen out of medical and/or clinical care
• People newly diagnosed with HIV
• Clients who have missed appointments, lab work or screenings
• Clients who need support sticking to their treatment plan
• Clients who have unsuppressed viral loads or poor CD4 blood cell response to antiretroviral therapy

Outreach services connect clients to their care teams and may include:

• Home visits
• Phone calls
• Case conferencing with medical providers
• Connecting with other network members

Our outreach aims to improve health outcomes for disproportionately affected populations. This includes communities of color, transgender clients, pregnant people, and individuals who struggle with substance use, mental health issues, and/or homelessness.

Partner Counseling & Referral Services (PCRS)

• PCRS helps people living with HIV or AIDS talk to their current or past partners about their HIV status and the possibility of exposure. PCRS can also provide partners with information about HIV testing, prevention, and treatment.
• PCRS is free, voluntary and confidential.
• Anonymous partner notification services are available.
