A guide for Contra Costa Regional Medical Center & Health Centers patients

Schedule an Appointment Online

Contra Costa Regional Medical Center & Health Centers' patients with MyChart accounts can make appointments online to see their primary care doctors. If you don't have a MyChart account, sign up for one online, call us toll-free at 1-844-622-5465 or email mychart.support@cchealth.org to get an activation code.

7 a.m. to 6 p.m., Monday - Friday

800-495-8885 is the central number for making appointments at our county health centers. For same-day or next-day appointments, it's best to call as early as possible. If it can wait, we recommend calling in the afternoon when hold times are much shorter. Please note that most appointments cannot be scheduled further than four weeks out. Members of the Community Provider Network can call the community clinic where they receive care to schedule an appointment.

Arrange One With Your Doctor or Nurse

If your provider wants to set up another appointment within four weeks of your visit, they can schedule it at the clinic.

Specialty Appointments

Most specialty appointments require a referral. Referrals to see specialists are often made by your primary care provider.

Lab Services

Your provider may ask you to go to one of our labs to have a blood test or provide some other type of bodily sample, such as a urine sample. We provide lab services at nearly all of our outpatient health centers.

Canceling an Appointment

Call the appointment unit at 800-495-8885 and leave a message saying you want to cancel your visit. You may also speak with an agent if you wish to reschedule your appointment. Please call at least 2 hours ahead of time. Appointments can also be canceled online through MyChart.

Getting Care After Hours

If you get sick or hurt after regular office hours, you can still get medical care. In an emergency you may call 911 or go to the nearest Emergency Room (ER), including the ER at Contra Costa Regional Medical Center in Martinez. If you aren't sure if you need to go to the ER, call the County's free 24-hour advice nurse to help you decide (1-877-661-6230). The advice nurse can tell you what to do before you go and what to tell the emergency staff once you get there.
