What is Contra Costa Health Plan?

Contra Costa Health Plan (CCHP) is an HMO, or health maintenance organization. HMOs cover health services provided by a network of specific doctors and providers. In Contra Costa County, CCHP is the primary managed-care plan for people with Medi-Cal. You must first enroll in Medi-Cal to become a CCHP Medi-Cal member. CCHP also offers managed care to County employees and IHSS workers.

CCHP covers the cost of healthcare services for its members, including primary care, specialty care, and more.

CCHP currently provides health or dental coverage for more than 20% of Contra Costa County residents, including one in three children.

    • CCHP addresses every aspect of care, including primary care (regular doctor's visits), specialty care for specific health needs, mental and behavioral health services, and more.
