A primary care doctor or provider (PCP) is a key part of staying healthy. PCPs care for your general health, provide information on health topics and make referrals to specialists when needed.
For Contra Costa Health Plan Members
If you're a Contra Costa Health Plan member, you can use the provider directory to search for a primary care doctor or specialist near you that serves CCHP members.
If you'd like to change your primary care provider, you can do so easily:
- Find a new doctor using the provider directory
- Then, fill out the online primary care provider change form
Finding a Doctor Outside of Contra Costa Health Plan
For People With Insurance Other Than CCHP
Talk to your health insurance provider for help finding a primary care doctor who will accept your insurance.
For People Without Insurance
If you don't have health insurance, you can call the financial counseling line at 800-771-4270 to learn about health coverage options.
