Your Primary Care Provider (PCP) is important!

Your Primary Care Provider (PCP) takes care of your general health care needs. PCPs give you regular checkups to monitor your health and make sure you stay healthy. Your PCP also helps coordinate most of the other medical care you get, like certain labs you may need or care from a specialist.

If you are a CCHP Medi-Cal member, Contra Costa Health Plan will assign you a PCP if you do not pick one.

Visit your PCP

You can find your assigned PCP or Care Team members in the MyChart member portal or call us at 1-877-661-6230 (TTY 711). You can sign up for a MyChart account if you don't already have one. You should make an appointment with your PCP within 120 days after joining CCHP. Children under 18 months should be seen within 60 days for routine check-ups and immunizations. Pregnant members should be seen within 2 weeks of joining CCHP.

Changing your PCP

CCHP members can change their PCP any time. Just fill out our online form or call CCHP Member Services at 1-877-661-6230 (Option 2) (TTY 711 for hearing or speech impaired). You can search for a new PCP by location, gender, language, or other features.

Medi-Cal members may choose a PCP from the Regional Medi-Cal Center (RMC) network or Community Provider Network (CPN). Medi-Cal members who meet one of the Kaiser criteria also have the option to request to get all their Medi-Cal managed care services through Kaiser using the same online form or by calling Member Services.

Plan B members may choose a PCP from the Regional Medi-Cal Center (RMC) network or Community Provider Network (CPN).

All other members may choose a PCP from the Regional Medical Center (RMC) network.

Members with other primary health coverage can see a PCP that is allowed under their primary insurance. Members will be restricted to getting care through a CCHP plan provider only if a service or benefit is not covered under the other primary health coverage, and is only covered under the CCHP plan.

Continuity of Care - Keeping Your Old Provider 

  • If you had previously been seeing a provider that is not in CCHP's network and you want to continue to see that provider, ask CCHP for "Continuity of Care."
  • Just call Member Services, Monday - Friday, 8 a.m. - 5 p.m., at 1-877-661-6230 (Option 2) (TTY 711 for hearing or speech impaired).
  • CCHP will review your case and let you know if you are approved to continue to see your previous provider for a certain amount of time (up to 12 months), and have it be covered by CCHP.
