Medication-assisted treatment for opioid addiction

The Choosing Change clinic provides treatment and empowers people who want to stop using opioids. Patients receive dedicated support from a compassionate and knowledgeable medical team. 

The clinic offers a holistic approach to treatment:

  • Choosing Change patients use buprenorphine (also known as Suboxone), which reduces craving and withdrawal symptoms.
  • Patients receive group therapy and medical visits from a team of doctors or nurse practitioners, nurses, mental health counselors and community health workers.
  • Patients meet one-on-one with their care team for support, counseling and connections to other recovery services.

How to Join

Choosing Change is an option for people who have been diagnosed with Opiate Use Disorder (OUD) and want to stop using.

This program is available to patients with Medi-Cal, Contra Costa Health Plan, or Medicare, including Part D. If your insurance is different, please talk to your healthcare provider about other treatment options.

  • Call the Behavioral Health Access Line at 1-800-846-1652, or
  • Talk to your medical provider to see if you qualify.

Schedule

Once you're enrolled in Choosing Change, appointments and group medical visits are regularly available via telehealth and in-person at the following Contra Costa locations:

  • Antioch Health Center
  • Brentwood Health Center
  • Concord Health Center 
  • George & Cynthia Miller Wellness Center (Martinez) 
  • Pittsburg Health Center
  • West County Health Center (San Pablo)

Groups are scheduled by appointment and walk-ins may not be available.
