Contra Costa Continuum of Care
The Contra Costa Continuum of Care (CoC) unites over 50 organizations, cities, towns and individuals with a shared mission to prevent and end homelessness in Contra Costa County.
As the lead agency, Contra Costa Health:
- Oversees the Coordinated Entry System
- Manages the Homeless Management Information System (HMIS) database
- Collaborates with various homeless service providers
What is a CoC?
Funded by the U.S. Department of Housing and Urban Development (HUD), the CoC strategically plans housing and services for people experiencing homelessness.
CoC Goals:
- Ending homelessness
- Funding rapid rehousing
- Promoting program access
- Optimizing self-sufficiency
