Care Coordination

  • Triage medical reports received by CHDP to the appropriate level staff
  • Provide appointment scheduling, authorizations and referral for children with medical and/or dental problems
  • Obtain regular preventative health assessments for infants, children and adolescents
  • Refer children with suspected health problems for necessary diagnosis and treatment
  • Coordinate with other providers for follow-up of identified health problems, and
  • Help children find medical and dental homes for their health care by maximizing enrollment of uninsured children into an appropriate health insurance program

Dental Care Coordination

Beginning January 1, 2020, CHDP will offer care coordination services to Medi-Cal dental (Denti-Cal) providers for fee-for-service children who are lost to care. The goals of these new services are to:

  • Minimize the number of Medi-Cal members ages 0-6 years of age who have been referred or scheduled for a dental appointment who have not had dental follow-up /care.
  • Minimize the number of medi-cal members under 21 years of age with an urgent /emergent dental condition who have not had dental follow up/care.
  • Educate and assist Medi-Cal members whose care is being followed by CHDP to establish a dental home.

Provider Relations

Recruitment, retention and quality assurance activities for providers participating in the CHDP Program.

Resources: Local CHDP staff provides technical assistance and support to providers on program standards, billing issues and a variety of children’s health issues including childhood obesity, asthma, and oral health.

Trainings to CHDP providers in addressing:

  • Nutrition related issues:
    How to counsel the overweight child, growth assessment (BMI), lab screening & referrals to community resources.
  • Audiometric and Vision Screening:
    Annual provider training sessions for certification of staff.
  • Fluoride Varnish Training:
    • CHDP now offers a one hour in-office training for your staff, including a brief powerpoint training and hands-on practicum. To schedule a fluoride varnish training at your site, please email chdp@cchealth.org or call 925-608-5231 to coordinate services.

Act as a liaison between providers, state CHDP and Medi-Cal managed care plans.

Certification: Enroll providers into CHDP, perform certification of providers every three years, keep providers informed of new/revised CHDP state policies, & provide support to providers on patient and billing questions. Educate and promote CHDP and Gateway provider network.

  • Head Start Health & Nutrition Advisory Committee

Health Care Program for Children in Foster Care

The Contra Costa County CHDP program also administers a public health nursing (PHN) program for children in foster care known as the Health Care Program for Children in Foster Care (HCPCFC). PHNs identify and prioritize the follow-up on health services to include: medical, dental, mental, and developmental health needs. CHDP staff assist with the coordination of health services by making referrals to the appropriate community agencies such as: CHDP providers, dental providers, Regional Center, mental health services, California Children Services, and specialty providers.

  • Public Health Nurses serve as consultants to Social Service and Probation staff
  • Participate in Team Decision Making and SMART Team Meetings
  • Educate and train Foster Parents, Group Home and ILSP (Independent Living Skills Program) on issues related to health
