The Contra Costa Health Transitional Housing Program offers temporary housing for homeless youth aged 18-24. Residents work with our team to create personalized plans, focusing on education, employment, life skills and more.

  • Staffed 24/7

Transitional Living Apartments (West County)

  • For youth ages 18-24
  • Independent living program
  • Program lasts up to 24 months
  • Focus on independent living readiness
  • Onsite support services
