Public Health Nursing Services
Public Health Nurses serve the county in many ways.
Services are directed towards individuals and their families, groups and the community with special attention to the most at risk populations for health disparities.
Public Health Nurses provide:
- Comprehensive case management for perinatal, pediatric and adult clients
- Investigation and treatment of communicable disease
- Promotion of self-efficacy
- First response in disasters

PHNs improve the health and well-being of the community by focusing on many of the important health indicators:
- Access to health services
- Preventative services
- Environmental quality
- Injury and violence prevention
- Maternal, infant and child health
- Mental health
- Nutrition, physical activity and obesity
- Oral health
- Reproductive and sexual health
- Social determinants
- Substance use
- Tobacco use
Public Health Nurses work closely with other members of the health team in community based organizations, the faith community and neighborhood organizations.
Public Health Nurses work to improve the general health of the community through an integrated approach that considers ways to improve the physical, emotional and socio-economic well-being for all.
Public Health Nurses (PHNs) are Registered Nurses (RNs) who have a bachelor’s degree and a California Public Health Nurse certificate. A Public Health Nurse looks at the health needs of the community as well as the health needs of the individual.
The PHN seeks to promote health and prevent disease, disability and premature death. The PHN strives to improve the quality of neighborhood life by working in partnership with the community residents to create the conditions that ensure healthy lives.
